Admission Procedures



College


College Freshmen


Steps:

  1. Present Form 138 at the Admissions Office
  2. Pay processing fee at the Finance Office
  3. Get Admission Kit at the Admissions Office
  4. Fill out HAU-ADM Form #1 (CET Application Form)
  5. Get CET schedule and take the CET
  6. Have HAU-ADM Form #3 (Recommendation Form) filled out by HS Guidance Counselor or by the class adviser
  7. Go to the University Guidance Center for test result and individual interview with Guidance Counselor
  8. Submit the following documents in HAU yellow envelope at the Admissions Office:
    • College Application Form
    • Original Form 138
    • Birth/Baptismal Certificate (photocopy)
    • Marriage Certificate (if married)
    • Undertaking
  9. Get Permit to Enroll
  10. Proceed To Enrollment

Transferees


Steps:

  1. Present the following credentials to the Admissions Officer:
    • Transfer Credentials
    • Certified True Copy of Grades for evaluation purposes
  2. Initial interview by the Admissions Officer, Dean (for Accountancy, CICT, CASED, Nursing students only)
  3. Pay processing fee at the Finance Office
  4. Get Admission Kit at the Admissions Office
  5. Fill out CET Application Form
  6. Get CET schedule and take CET
  7. Interview with Guidance Counselor (College Guidance Center)
  8. Interview with Student Conduct Officer
  9. Submit the following documents at the Admissions Office:
    • College Application Form
    • Transfer Credentials
    • Certified True Copy of Grades
    • Certificate of Good Moral Character
    • Barangay Clearance
    • Birth/Baptismal Certificate
    • Marriage Certificate (if married)
    • Undertaking
  10. Get Permit to Enroll
  11. Proceed To Registrars Office
  12. Proceed To Enrollment

High School


High School Freshmen


Steps :

  1. Present photocopy of card (with grades in the third grading period) at the Admissions Office.
  2. Pay processing fee at the Finance Office.
  3. Present receipt at the Admissions Office and get admission kit. Fill out Application Form and get the HAUHSET schedule.
  4. On the scheduled date take the HAUHSET.
  5. Get HAUHSET result at the Guidance Office on the specified date. Those who qualify will be given orientation schedule and pass.
  6. Attend orientation with parents.
  7. Get permit to enroll.
  8. Proceed to enrollment.

High School Transferees


Steps :

  1. Present original copy of report card with a general average of at least 80% and no failing grade in the fourth grading period to the Admissions Office.
    • Transfer Credentials
    • Certified True Copy of Grades for evaluation purposes
  2. Initial interview by the HS Guidance Counselor.
  3. Pay processing fee at the Finance Office.
  4. Present official receipt at the Admissions Office and get Admission kit.
  5. Fill out application form and get HAUHSET schedule.
  6. Take the HAUHSET on the scheduled date.
  7. Get HAUHSET result at the Guidance Office. Submit recommendation for and certificate of good moral character during the final interview by the HS Guidance Counselor. Get orientation schedule and pass.
  8. Attend orientation with parent or guardian. Sign and submit the undertaking.
  9. Get permit to enroll
  10. Proceed to enrollment.

Elementary


Kinder I, Kinder II, Grade 1


Steps :

  1. Present photocopy of both birth certificate and baptismal certificate at the Admissions Office.
  2. Pay processing fee at the Finance office.
  3. Present receipt at the Admissions Office and get admissions kit. Fill out application form. Kinder applicants must proceed to the ELS Principal’s office.
  4. Get HAULSET schedule and take the test on the scheduled date.
  5. Get the test result at the ELS guidance office on the specified date given by the HAU Institutional Testing Center.
  6. If qualified, submit the requirements at the Admissions office
  7. Attend orientation with parent or guardian.
  8. Secure permit to enroll.
  9. Proceed to enrollment.

Grade School Transferees


Steps :

  1. Present photocopy of card (3rd grading period) with a general average of at least 83% and conduct must not be below 85% at the Admissions Office.
  2. Pay processing fee at the Finance Office.
  3. Present receipt at the Admissions Office and get admission kit. Fill out application form
  4. Get HAULSET schedule and take the test on the scheduled date.
  5. Get the test result at the ELS guidance office on the specified date given by the HAU Institutional Testing Center.
  6. If qualified, submit the requirements at the Admissions Office
  7. Attend orientation with parent or guardian.
  8. Secure permit to enroll.
  9. Proceed to enrollment.

Graduate School


New Students


  1. Present the original and photocopy of transcript of records for evaluation and submit 1 (one) copy of 1.5 x 1.5 latest photo at the Graduate School Office (GSO).
  2. Pay the processing fee of P350.00 at the Finance Office.
  3. Proceed to the Admission’s Office with the original TOR signed by the evaluator, processing fee receipt and 3 (three) copies of 1.5 x 1.5 latest photo. An application kit will be given to the student. The student will fill up an application form. Together with the application kit are the recommendation letters (for GSB student only). These are form letters to be filled up by the immediate superior and former teacher. The student will be scheduled for an Admission Exam.
  4. The student will then be notified by the Graduate School Office if he/she passed the admission exam and will be scheduled for an interview with the Dean/Program Coordinator.
  5. Submit all the requirements at the Admission’s Office and get a Permit to Enroll.
